Do you have questions on the new insurance???? The first step is to call the numbers posted below. If you still have unresolved questions after contacting customer service, please talk to Donella Belleau so that she may submit the proper forms to the state. Additionally, those who are still without cards need to contact Donella Belleau.

Delta Dental: 800-234-3375

Caremark: 800-294-6324

Remember:
You can qualify for Health and Dental insurance at the part-time rate if you work 40 hours a pay period. You qualify for Health and Dental insurance at the full-time rate if you work 72 hours a pay period.
